National Teleconference: Two Key ACE Interventions: patient-centered interdisciplinary care & medical care review

The American Geriatrics Society- Acute Care special interest group announces a national telephone conference on "Two Key ACE Interventions: patient-centered interdisciplinary care & medical care review"", on Tuesday, January 17, 2012, from noon - 1pm, Central Time. This will be a conference call to learn about how featured ACE Units are designed and the challenges they face. National experts will help us to discuss strategies to address the obstacles. No registration is required and there is no charge.

The featured ACE programs will be San Francisco General Hospital/UCSF, Dr. Edgar Pierluissi & Dr. Margarita Sotelo and the University of Texas-San Antonio/Christus Health Care, Dr. Liliana Oakes.  

Dr Robert Palmer, Eastern Virginia Medical School, and Denise Kresevic, RN Ph D GNP-BC, CNS-BC, University Hospitals Case Medical Center in Cleveland, will provide their Summary Teaching Points.

The Goal:  This will be a national telephone conference call to hear about the challenges of developing and managing an ACE program.

When? January 17, 2012, 12 noon until 1 pm (Central Time).

Content:  
1) Background information briefly describing each hospital, community, and health care system.
2) The issues / challenges that each ACE program was trying to address.
3) A description of each ACE unit patient-centered interdisciplinary care.
4) A description of Medical care review on each ACE unit.
5) The key results thus far.
6) Issues that others may wish to consider to maintain fidelity to the ACE model of care.
7) The biggest challenges at this stage of the project.


Who may benefit from the session?  Members of the Canadian Geriatrics Society; members from the American Geriatrics Society; members of the Medicare Innovations Collaborative; hospital leaders interested in hearing more about geriatrics models of care; geriatricians and nursing leaders who lead ACE units; geriatrics faculty members and fellows: NICHE members.

The format:  Michael Malone will facilitate the session and Aurora Health Care will host the call. Two successful ACE units from America (UT San Antonio and UCSF) will each have a 10-12 minute formatted, oral description of their programs (about 10-12 slides).  Dr. Robert Palmer and Denise Kresevic will each provide a 5-7 minute summary teaching point discussion, as faculty experts on the ACE model of care. Dr. Malone will help to facilitate a discussion / question period for participants. Follow up information including a bibliography and an audio file of the session will be provided for participants. This is the third of four leadership sessions we propose in anticipation of the next AGS meeting in Seattle, Washington on May 2-5.

Learning objectives: To understand how healthcare professionals on ACE units provide patient-centered interdisciplinary care and how they review the goals/ plan of care for older patients.
